| Abstract;A 78-year-old man visited our hospital because of a left lower quadrant mass. On digital rectal examination, an enlarged cystic mass having the characteristics of a smooth surface and soft elasticity was palpated at the left side of the prostate. CT scan and MRI demonstrated a prostate with a cystic mass, 8cm in diameter, and large masses consisting of multicystic and solid lesions in the left retroperitoneum. The serum concentration of prostate-specific antigen(PSA) was significantly elevated to 5,263.7ng/ml. Percutaneous abdominal needle biopsy revealed the histopathological features of moderately differentiated adenocarcinoma, and positive immunohistochemical staining for PSA. Hormonal therapy with flutamide and LH-RH agonist induced a significant improvement in serum PSA level and the reduction in the size of the masses in the retroperitoneum. This was thought to be a rare case of prostatic cancer forming a cystic component not only in the primary, but also in the metastatic lesion. (author abst.) |
