| Abstract;A 34-year-old man with hypertension had been diagnosed with pheochromocytoma, and underwent adrenalectomy in July 1989. However, hypertension and a high level of cathecholamine in the urine continued after the operation. Moreover, difficulty in urination and walking appeared in April 1990. CT scanning showed lymph node swelling, while abnormal high uptakes in the cervical and lumbar spine were detected on bone scintigram. He was diagnosed with malignant pheochromocytoma and referred to us. After he had received 10 courses of cyclophosphamide, vincristine and dacarbazine(CVD) therapy and 8 courses of VP-16 therapy, his cathecholamine level in urine decreased, and subjective urinary and walking conditions returned to normal. The hypertension was well controlled by .ALPHA.-methyl-tyrosine(.ALPHA.-MT) which is a tyrosine hydroxylase inhibitor. However, 1 year and 2 months later, malignant pheochromocytoma recurred in systematic lymph nodes and bone, and he died 4 years and 8 months after the adrenalectomy. In this case, CVD therapy and VP-16 therapy showed temporary effectiveness against malignant pheochromocytoma. (author abst.) |
