Abstract;A 76 old Japanese female who underwent simple surgical excision and radio-therapy three times before for a medial canthal right lower Iid basal cell carcinoma was admitted to Kaisei Hospital Eye Clinic upon recurrence of the tumor in the right nasomaxillary region. Orbital CT scan, X-rays and blood examinations did not reveal orbital or distant metastasis. A biopsy from the lesion reconfirmed the diagnosis of basal cell carcinoma. Surgical excision with a safety margin exceeding 3mm and artificial skin grafting was followed by a lower lid reconstruction with a split-thickness skin graft from the right abdomen. Intraoperative frozen section controls were taken and were free of tumor cells. We believe that eye lid basal cell carcinomas located around the medial canthus and medial nasomaxillary region tend to be agressive and reccur. We also think that total surgical excision taking adequate surgical margins and split thickness grafts from the abdomen in patients similar to our case may yield cosmetically and functionally acceptable results.
