Abstract;In order to evaluate the effects of CS-905 (azelnidipine) on the diurnal variation of blood pressure, 39 inpatients with mild to moderate essential hypertension were treated with 1-8mg of CS-905 once daily after breakfast (once daily group) or divided into two doses given after breakfast and dinner (twice daily group). The results were as follows: 1) The decrease of blood pressure was maintained stably over 24 hours in both groups. 2) Treatment with CS-905 did not increase the variability of blood pressure in any patient from either group. Neither the amplitude of changes nor the diurnal variation of blood pressure was affected by either dosage, nor was there any effect on the pulse rate. 3) The treatment achieved a hypotensive effect rating of as "Effective" in 72.7% and 93.8% of the once and twice daily groups, respectively. 4) Adverse effects occurred in two patients receiving CS-905 once a day; one developed a skin rash and pruritus, while the other had elevation of GPT and ALP. These adverse effects were mild and were not considered to be of major clinical significance. From these results, CS-905 (azelnidipine) seems to be a useful antihypertensive drug, which has a stable hypotensive effect without altering the diurnal variation of blood pressure and pulse rate when administered once a day.
