| Abstract;This 4-year old boy(weight 16kg) was transferred to our hospital by ambulance immediately after traffic accident. The patient was comatose(Glagow Coma Score of 6) on admission without focal neurological deficit. Computed tomography(Fig.1) revealed small hemorrhage within the quadrigeminal cistern and the right basal ganglia. Magnetic resonance imaging(Fig.2) also demonstrated the lesion on the corpus callosum. The patient was diagnosed as diffuse axonal injury and underwent conservative treatment. The patient's consciousness level had not recovered for 11 days and electroencephalogram(Fig.3) showed a tendency to have slow waves and dysrhythmia. Intravenous administration of thyrotropin-releasing hormone(TRH) was initiated for the treatment of prolonged coma. After20 days of TRH administration, the patient showed a remarkable improvement in both of consciousness level and EEG findings(Fig.4,5). The patient was discharged without significant neurological deficit on the 41st day.Although there have been few reports regarding TRH administration for the treatment of comatose infants and dosage and the period of administration of TRH were still unknown, this is the case in which intravenous administration of TRH at the dose of 0.5mg/day(about 30.MU.g/kg/day) for 20 day seemed effective in the treatment of comatose infant with diffuse axonal injury.
