| Abstract;A case of ureteral stone leading to multiple organ failure is reported. A 56-year-old female was admitted to our hospital with the complaint of high-grade fever, nausea, oliguria and jaundice. Physical examination and laboratory data revealed septic shock, acute renal failure, bilirubinaemia and dsseminated intravascular coagulation(DIC). KUB revealed a calcific shadow at the left side of the 3rd lumbar vertebra. Ultrasonogram demonstrated left hydronephrosis due to incarceration of ureteral stone. Left percutaneous nephrostomy was placed both for drainage and to proserve renal function on the 1st hospital day. Antegrade pyelogram demonstrated a ureteral stone 3mm in diameter with complete obstruction of the left ureter. In spite of the nephrostomy being placed, renal function did not improve, and the patient needed transient hemodialysis. In addition, on the 2nd hospital day, she developed adult respiratory distress syndrome(ARDS) and needed a respirator for thirteen days. She recovered from renal failure after undergoing hemodialysis for the 6th time, and her serum bilirubin level decreased significantly to within the normal range. After recovering from multiple organ failure, transurethral lithotripsy was performed. At present, 11 months after the operation, no recurrence of urinary calculus has been seen, and the patient has retained renal function. (author abst.) |
