| Abstract;The case was a 51-year-old male with type-2 diabetes with autonomic neuropathy, preproliferative retinopathy and mild hypertension. The duration of diabetes was 20 years. His hyperglycemia was poorly controlled with insulin. No abnormal finding was found on plain chest x-ray, however, electrocardiography indicated left ventricular hypertrophy and myocardial ischemia without clinically evident heart disease. In addition, M-mode ECHO and pulsed Doppler cardiography indicated abnormalities of both the left ventricular systolic and diastolic functions. Although no significant stenosis of major coronary vessels was found on angiography, the wall motion of the left ventricle was diffusely impaired. Diagnosis of diabetic cardiomyopathy was made by the results of these examinations, and cardiac muscle biopsy was performed, which proved arteriolar wall thickness with hyalinization and luminal stenosis. After having dinner on Dec. 31, 1995, he died suddenly while watching TV at home. Although the precise mechanism of the death was not clear, acute pump failure (cardiac arrest) or ventricular fibrillation due to cardiomyopathy may have been the cause of sudden death. These findings suggest that diabetic cardiomyopathy, like idiopathic cardiomyopathy, may be a possible cause of sudden death in diabetic patients. (author abst.) |
