Abstract;In this case report, we present a 17-year-old female who developed hallucino-delusional symptoms after undergoing an operation for an arteriovenous malformation (AVM) in the left supra-medial temporal lobe. She had been suffering from visual hallucinations and delusions of persecution for 3 years after the AVM operation. She was first diagnosed to be schizophrenic. Her insight into the hallucino-delusional state was well preserved and the visual hallucinations were deemed to be a transient phenomenon. We diagnosed her symptoms to be psychic seizures (simplex partial seizures) caused by the epileptogenic focus on the left temporal lobe. During the interictal state, we observed spike discharges from the left mid-temporal area on EEG, and hypoperfusion in the same area with SPECT. This case thus provided us with valuable insight regarding the differential diagnosis between schizophrenia and psychic seizures. (author abst.)
