| Abstract;Cecal volvulus occasionally occurs in patients with mental disorders treated with habitual sedative or those in long-term bedrest, although the cecum is a relatively uncommon site for volvulus, accounting for 5.9% of all colonic volvulus in Japan. Eight patients of 160 severely disabled adults in our hospital were operated for acute abdomen from September 1994 to May 1999. Among them, there were three cases of cecal volvulus, in which the incomplete fixation of cecum and ascending colon was confirmed at laparotomy. The incidence of cecal volvulus seems to be higher in serevely disabled adults than in control because of chronic constipation caused by long-term bedrest and anticonvulsive drugs. There are no specific abdominal symptoms to cecal volvulus. Furthermore, due to the difficulty in communicating with these patients and in taking appropriate abdominal signs and sufficient radiographic information, the preoperative diagnosis of cecal volvulus may be difficult. Therefore, the cecal volvulus should be included in the differential diagnosis of the bowel obstruction in severely disabled adults. (author abst.) |
