| Abstract; We have many opportunities to examine young patients whose chief complaint is back pain or lower back pain. Because the pain has various causes it is difficult to diagnose the exact cause. We have recently experienced two cases of osteoid osteoma resulting in back pain, the other in lower back pain. One case was osteoid osteoma in the thoracic vertedra of a 29y.o. man, the other in lumbar vertebra of a 16y.o. man, This report is of the diagnostic and therapeutic courses followed in these cases. On the first visit both cases, we found no abnormal neurological signs except localized pain at the vertebra when tapped lightly. In the first case (a 29y.o. man), the X-ray showed no abnormality. In the second case, the X-ray revealed an abnormality after 6 months. While in both cases pain was experienced, night pain which is common in osteoid osteoma cases, was not. Asprin which is known to effective for osteoid osteoma pain relieved the discomfort. Osteoid osteoma in vertebrae is not frequent, slow to progress and, difficult to diagnose in the early stages. As a result, the clinical course tends to be elongated. Therefore, we should keep this in mind as a possible cause of back pain or lower back pain in adolescence, especially when there are no inflammatory signs and rest treatment has no effect.
