Abstract;[Introduction] Even though some commonly used atypical antipsychotics are more effective than other typical drugs, parenteral haloperidol therapy is still useful in treating patients in an acute phase. Many clinicians administer haloperidol injections in expectation of a rapid effect. However, some case reports have described the development of suicidal behavior due to haloperidol-induced akathisia. We therefore investigated the conditions and results of injecting haloperidol in patients being treated at a hospital. [Methods] The subjects of our study consisted of out-patients and in-patients who received either intravenous or intramuscular haloperidol at Nagasaki University Hospital from 1988 to 1994. We retrospectively investigated the background data, administration route, primary effects, side effects, and suicide attempts reported based on the patients' medical records. [Results and Conclusion] The patient population of our study was 139 (78 males, and 61 females). The age distribution was 15 to 79, with an average age of 32.1 years. The total frequency of injection was 651 with a total amount of 3942.8 mg. The frequency of a single intravenous shot was 199, while DIV or IVH was given 83 times, and intramuscular was given 369 times. Regarding the effects of 579 injections of haloperidol, 539 (93%) induced more than a slight improvement in the patient condition. In 10 cases (7.1%), side effects were observed following haloperidol injection. These included increased levels of serum creatinin phosphokinase, acute dystonia, fever, muscle rigidity, tremors, malignant syndrome, and akathisia. No cases attempted suicide as a result of these side effects. In the course of treatment (average follow-up period, 4.3 years), 7 patients attempted suicide and 2 did so within one month after receiving haloperidol. However, the latter 2 patients had also frequently attempted suicide prior to their haloperidol treatment.... (author abst.)
