Abstract;In the pathogenesis of atopic dermatitis (AD), it is considered that chronic skin inflammation mediated by T-cells plays a critical role. For the causative antigens of AD, common substances taken daily into the human body must be suspected. All over the world, but especially in the industrialized countries, metals have been widey used for human dental treatment, expect for babies. Don't dental metals, as well as, house dust mites (HDM) play a major role in the pathogenesis of AD? In this study, we performed antigen-specific lymphocyte stimulation tests (LST) to determine the causative antigens of AD patients instead of skin patch tests. For 350 AD patients, ages ranging from 8 to 70, and 10 healthy controls, LST to dental metals and HDM were performed. As allergens for LST, we used soluble metal salts (Hg, Ni, Au, Ag, Pd, In and Ti), purified Der. f antigen and resins. A stimulation index (SI) of more than 180% was regarded as a positive reaction. Positive LST reactions were obtained from Hg in 84% of the patients, Ni in 87.9%, Au in 18.8%, Ag in 11.2%, Pd in 15.5%. In in 11.4% and Ti in 58.1%. The positive LST rates to resins were from 7.7 to 22.2%, to ionomer cement powder was 69.2% and to inonmer cement liquid was 33.3%. The LST to HDM was positive in 77.5% of the patients. Although we found no statistical difference in the LST positive rates between AD patients and healthy controls, 95% of the patients improved within one year without topical corticosteroids after the removal of the LST suspected causative metals and the critical reduction of HDM allergen with mite-free bed lines sets. Not only for the prevention, but also for the treatment of AD, it is important to prevent tooth decay from earliest childhood and to reduce HDM allergen. The safety of materials, including dental metals and resins used in treatment should be reconsidered from the immuno-allergological point of view as soon as possible.... (author abst.)
