| Abstract;We treated a 73-year-old man with Legionella pneumonia, whose life was saved due to early detection of the disease by the urinary antigen test. A fever of 38.5.DEG.C. was noted on May 6, 1996. He was then admitted to a nearby hospital for treatment. However, because of rapidly worsening respiratory function, he was transferred to our Critical Care Center on May 9 for intubation and artificial ventilation. Panipenem/betamipron was administered empirically for serious pneumonia; however, no causative organism could be detected by Gram's staining of the sputum. Administration of erythromycin and rifampicin was also begun as a precaution against possible Legionella pneumonia. The diagnosis of Legionella pneumonia was confirmed by a positive urine antigen test performed 2 days after admission, and the antimicrobial regimen was thus continued. When the chest X-ray findings and clinical test results improved soon thereafter, the patient was transferred to the general ward. Our experience suggests that when a clinician suspects the possibility of Legionella pneumonia, appropriate treatment should be initiated from an early stage, and necessary clinical tests, such as the urine antigen test, must be performed.
