Abstract;Generally, it is well recognized that clinical records is important. However some deficit is often observed in medical records. To obtain correct clinical information, we examined essential factors which might affect occurrences of deficits in admission medical records. We in this paper discussed what problems should be solved at the moment to attain a quality of a hospital information system or an electronic patient record system. (author abst.)
