Abstract;We compared 5 insulin immunoassay kits in the diagnosis of insulinoma; 3 (E, S, and L) use polyclonal antibodies and 2 (R and EN) monoclonal antibodies. Interference by proinsulin was highest in kit L, followed by kits E and S, and low in kits R and EN. In noninsulinoma cases, IRI was similar among kits, although that assayed using kit EN was 5-15% lower than that by other kits. In insulinoma patients, IRI differed considerably among kits. IRI by kits R and EN were similar. That by kits E, S and L were 3.7, 3.5, and 5.1 times higher than those by kits R and EN. Positive Fajans' index was thus affected. Lowering the cutoff of Fajans' index did not have much effect. When kit R or EN is used, separate determination of proinsulin is strongly suggested when screening insulinoma. (author abst.)
