| Abstract;Gastroesophageal reflux(GER), defined as an involuntary reflux of gastric contents into the esophagus, is a common disorder in infants and children. It can cause a variety of symptoms, such as vomiting, regurgitation, failure to thrive, heartburn, irritability, and hematemesis. Because of the diversity of its clinical presentation, the diaguosis as well as the choice of treatment in each individual is often challenging for general pediatricians. Guidelines for the clinical management of GER were, therefore, proposed by the working group of the European Society of Paediatric Gastroenterology and Nutrition in 1993. The North American Society for Pediatric Gastroenterology and Nutrition (NASPGN) organized a working group to develop new guidelines for GER, which were published in July 2001. These new guidelines are characterized by the policy to be evidencebased and by the detailed presentation of previous scientific studies for GER. They are also characterized by classification of patients with GER based on their clinical manifestations and by a complex algorithm to sggest the appropriate way of managing patients in each group. Although the NASPGN guidelines might be rather complicated, their clinical usefulness is promising for general practitioners including Japanese pediatricians. It is also expected that these guidelines will help us to develop new guidelines for Japanese practitioners. (author abst.) |
