| Abstract;The extra-glandular manifestations of patients with primary Sjoegren's syndrome (SS) have been reviewed with particular reference to the muscular and articular manifestations. Patients with SS often experience myalgias and muscle weakness but serum muscle enzyme levels are usually normal or slightly elevated. Thus it is thought that myopathy of SS is less severe than that of polymyositis/dermatomyositis. Most patients respond well to nonsteroidal anti-inflammatory drugs (NSAIDs). Articular signs and symptoms are the most common extra-glandular manifestations in SS. The arthropathy includes arthralgias, morning stiffness, intermittent syno-vitis and rarely chronic polyarthritis. Joint deformity is unusual and erosions in hand X-rays are rare. Treatment with NSAIDs results in improvement of arthralgias and arthritis. Administration of disease-modifying anti-rheumatic drugs (DMARDs) may be considered for the treatment of chronic polyarthritis although their therapeutic role in arthropathy of SS is unclear.
