| Abstract;Japan is facing serious childhood medical care problems including a crisis in the pediatric emergency-care system and overworked pediatricians. The systems in the United Kingdom (UK), United States (USA) and Germany do not seem to have so serious problems concerning above matters as in Japan. In UK and USA, emergency specialists take care of both adult patients and child patients in emergency outpatient units in hospitals. It is prohibited for emergency-care doctors to reject child patients even in a hospital without a pediatric emergency specialist. In Germany, pediatricians in training responsible for pediatric wards should cope with child patients coming to a hospital emergency room after working hours, but are free from provide care for duty when they are over 40 years of age or become a specialist pediatrician. To improve the pediatric emergency care system in Japan, it may be one of the solutions to create the specialist position in emergency medicine and train young doctors for the profession. The number of doctors in child health care in Japan is less than half that of the UK, the USA and Germany. Basically, it is urgent task for us to increase the number of pediatricians in Japan.
