| Abstract; This article examines a case of allogeneic bone marrow transplantation (BMT) in an acute myeloblastic leukemia patient colonized with class A vancomycin (VCM)-resistant enterococcus faecium (VRE). We did not try to eradicate the VRE during conventional chemotherapy even though the stool and urine cultures were repeatedly positive for VRE. At the time of BMT, we used oral norfloxacin (NFLX) that the VRE was still sensitive to. Then the VRE became negative for all specimens in the second week after BMT. VCM-sensitive and quinolone-low-sensitive enterococcus faecium were isolated from stools on the second day after BMT and replaced the VRE for the following months. The patient did not have VRE bacteremia during pre- and post-BMT. NFLX would be effective for eliminating VRE in the patient. For prevention of additional acquisition of NFLX-resistance, however, NFLX should he administrated in a short-period including both the first and second week after BMT when there is a high risk of VRE bacteremia.
