Abstract;[Objective] To evaluate the renoprotective effects of the candesartan cilexetil(Cand). [Design] Time-control study for 12 months in the outpatient clinic. [Method] 16 patients suffering from either chronic glomerulonephritis(CGN) or diabetic nephropathy (DMN) were divided into 2 groups. Among 14 CGN patients 11 were confirmed their pathological changes by renal biopsy. Group A, which consists of 9 CGN patients who had not been given any angiotensin coverting enzyme inhibitor (ACE-I) or Angiotensin II Receptor Blocker(ARB), were administered 4-8mg Cand and was checked their urinary protein(UP), creatinine clearance(Ccr), urinary N-acetyl-.BETA.-D-glucosamidase(NAG), and urinary .BETA.2microglobulin (.BETA.2MG) every 2-4 months. Group B, which consists of 5 CGN and 2 DMN patients who had been given ACE-I, was added 4-8mg Cand and were checked same factors. All patients had not taken steroids. Their other drugs, in which antiplatelet agents involved, were not changed in the obsevation period. [Results] Group A's UP was reduced significantly in 8-12 months period, however, Ccr was not changed significantly. NAG was reduced significantly in 8-12 months period, .BETA.2MG was reduced significantly between 3-5 months period and 8-12 months period. Group B's UP, Ccr, NAG, and .BETA.2MG changed little. Altough nearly significant, % reduction of UP and % reduction of NAG, a marker of a damage of proximal tubular cells, seemed to have a slight positive relationship in group A. [Conclusion] Recently AT2 receptor was discovered in human kidny interstitium and renal tubular cells. Thus it suggested that Cand has renoprotective effect through not only the dilect effects to the glomerular hemodynamics but through anti-fibrotic changes in interstitium via AT2 receptor activation.
