| Abstract;To clarify the pathological condition of RSV reinfection, we evaluated clinical manifestations and severity of illness in patients with RSV reinfection. From 1994 to 1996, 342 patients were given a diagnosis of initial RSV infection based on antigen detection in nasopharyngial secretions using the EIA method. One hundred and sixty-five of these patients were followed-up for three years from January 1996 to December 1998. When the patients first had respiratory symptoms, the presence of RSV in the nasopharyngial secretion was examinated. Among them, 41 patients were diagnosed as having second infection with RSV. We compared the clinical features and severity of illness between at initial and at second infection. The most common clinical entities at initial infection were bronchiolitis and pneumonia, and those at second infection were bronchitis and pharyngitis. Seventy one percent of the patients at initial infection and 22 percent at second infection required hospitalization. At initial infection, 16 of the patients required oxygen supplementation, but none of the patients at second infection required that treatment. All four cases with a third infection had lower respiratory disease. Our findings suggest that the severity of illness in RSV second infection is generally milder than that in RSV initial infection, but that in RSV third infection is not always mild. (author abst.) |
