| Abstract;Objective; To clarify the important factors for antidepressant medication in advanced cancer patients, we compared plasma concentration of nortriptyline of physically health patients and advanced cancer patients, and examined the relationship of CYP2D6 gene mutation to plasma concentration in advanced cancer patients. Subjects; Advanced cancer patients receiving nortriptyline were included. Former study in physically healthy subjects were used as control subjects. Method; After receiving nortriptyline more than 2 weeks, plasma concentration of nortriptyline was determined with high performance liquid chromatography. CYP2D6 mutations were identified using mutation-specific polymerase chain reaction amplification method. Result; Plasma concentration of nortriptyline was significantly higher in advanced cancer patients(n=8) than in physically healthy patients(n=41). But when adjusted by number of CYP2D6 mutation, difference was not significant. And difference of plasma concentration in advanced cancer patients by number of mutated alleles was not significant. Conclusion; High plasma level of nortriptyline was found in advanced cancer patients, but we could not find conclusive findings, because of small sample. More sampling is needed. (author abst.) |
