Abstract;The three points of 1) whether the diagnosis itself of the pneumonia is right, 2) whether the cause is a microorganism in which the antimicrobial agent does not work, 3) whether the factor which inhibits the effectiveness of administered antimicrobial agent does not exist, when the antimicrobial agent is invalid, were explained.To begin with, whether the diagnosis itself of the pneumonia is right must be reexamined, when the antimicrobial agent administered for the pneumonia is invalid, That is to say, the possibility of the pneumonia like shadow by the factors except for pathogenic microbe must be examined. There are not a few respiratory disease of noninfection presenting thorax radiological finding resembling to the pneumonia. And, the validity of the primary care is considered for the study on the following 4 items : Factors in the bacteria side, Factors in the host side, Factors in the drug side, and the problem in the effect measurement time. The approach of the differential diagnosis for the pneumonia like shadow of the antimicrobial agent invalidity was shown in the figure.
