| Abstract;A 59-year-old man admitted for poor plasma glucose control and further examination of multiple myeloma was diagnosed with type 2 diabetes mellitus 3 years earlier. Despite insulin therapy, his HbA1C fluctuated between 7% and 8%. He also suffered from multiple stage I myeloma, not requiring therapy. Hyperactivity, overeating, and weight loss occurred 4 months after discharge. Thyroid function was normal and other malignant diseases were excluded. Since HbA1C decreased to 4.8% with weight loss, insulin dosage was reduced to maintain good plasma glucose control. Eventually, insulin was not required to maintain plasma glucose control. Intrinsic insulin secretion gradually increased. He was diagnosed as manic. Body weight increased from 49 kg to 60 kg after lithium carbonate administration was started. Insulin secretion was then maintained and HbA1C was well controlled to 5-6% by diet alone. These findings suggest that a manic state and or abrupt weight loss may improve plasma glucose control in such cases as ours. (author abst.) |
