| Abstract;We evaluated treatments and therapeutic outcomes of idiopathic primary pneumothorax. The subjects were 59 patients, 67 sites (bilateral in 8 cases) who were initially treated in the hospital between January 1999 and December 2001. We treated then according to the following plans. Chest plain X-ray examination and chest CT scanning are conducted on admission to judge the presence or absence of bulla. The pleural cavity with pneumothorax is drained in principle. If the air leaks are stopped, the drain is removed on the third hospital day and the patient is discharged from the hospital on the next day. If the air leaks are not stopped, the video-assisted thoracoscopic surgery (VATS) is performed. If pneumothorx recurs, surgery is performed in all cases. Results: 1) As the initial treatment, bed rest was described in two cases and chest tube drainage was performed in 57 cases, 65 sites (bil. 8 cases). As for 43 sites, these patients were discharged from the hospital without surgery. Surgery was performed for 22 sites because the air leaks were not stopped. 2) The rate of recurrence by chest tube drainage alone was 27.9%. The rate of recurrence by the presence or absence of bulla was 40.7% in the presence of bulla or 11.7% in the absence of bulla. 3) As for surgical outcomes, VATS were performed for 35 sites, and the rate of recurrence after VATS were 8.6%. In conculsion, the operation is recommended for patients with initial full-blown pneumothorax, if the presence of bulla is evident. (author abst.) |
