| Abstract;In the treatment of colorectal carcinoma with intestinal obstruction, we have to relieve the obstruction and to provide a radical therapy for the malignancy. Left-sided colorectal carcinoma often causes intestinal obstruction. Its preoperative peroral decompression frequently yields poor results demanding emergency surgery, with unsatisfactory outcomes. The purpose of this study is to evaluate the clinical benefits of transanal decompression for such cases. Subjects were 32 patients undergone operations for obstructive left-sided colorectal carcinoma for the past 10 years, eighteen patients of them, from 1990 to 1994, were treated by preoperative peroral decompression (the early group) and 14 patients, from 1995 to 1999, were treated by peroral and/or transanal decompression before operation (the late group). The rate of elective operations, degree of curability and success rate in one-stage operation were compared between these two groups. The rate of elective operations increased significantly from 50% in the early group to 86% in the late group (p=0.039), and the rate of curative resection (the rate of curability A) changed from 61% to 86% (p=0.127). The rate of successful one-stage operation also increased significantly from 33% to 91%(p= 0.0045). In conclusion, preoperative transanal decompression for the treatment of obstructive left-sided colorectal carcinoma is considered to be very beneficial to avoid an emergency operation, to improve the rate of curative resection and to achieve more safe one-stage operation. Transanal decompression also contributes to the patient's QOL. (author abst.) |
