Abstract;We report a rare case of an incarcerated adult Bochdalek hernia successfully treated with an emergency operation. A 75-year-old woman was admitted to the hospital because of dyspnea, caused by diaphragmatic hernia. Symptomatic remission was attained by conservative therapy, but on the 46th day after her admission, she had sudden nausea and vomiting. Chest CT scan revealed that her left thorax was occupied with massive herniated small intestine and effusion, and that the lung was collapsed. Because she developed respiratory and circulatory failures, we performed an emergency operation under the diagnosis of an incarcerated adult Bochdalek hernia. Approximately 220cm of the small intestine was herniated through the dorsal defect of the left diaphragm, so called Foramen of Bochdalek. The foramen was about 5cm in diameter. We replaced the herniated small intestine, aspirated pleural effusion and closed the hernia opening. Postoperative course was uneventful, and no signs of recurrence have been detected. We think that surgical treatment is must be considered for an adult Bochdalek hernia. (author abst.)
