Abstract;We present a case of metastatic carcinoma of the rectum from gastric carcinoma mimicking primary rectal carcinoma. A 58-year-old woman, undergone a total gastrectomy for gastric carcinoma in May 1999, was admitted to the hospital because of gradually deteriorating constipation in December 2001. Histopathological study of her gastric carcinoma showed por, se, INF.GAMMA., ly1 v0 n2(+), stage IIIB, and Curability B. Ba-enema revealed entire stenosis of the rectum (Ra) and a CT scan showed wall thickening of the rectum. She was diagnosed as having primary rectal carcinoma and a low anterior resection was performed. Histopathology of the rectal tumor showed poorly differentiated adenocarcinoma that was similar to previous gastric carcinoma and grew mainly in submucosa. Definite diagnosis was metastatic carcinoma of the rectum from gastric carcinoma. Colonic metastasis from gastric cancer is relatively rare and unresectable in most cases. Sometimes we have great difficulty to differentiate it from primary colorectal carcinoma. It is required to diagnose based on detailed analysis of Ba-enema study and decide courses of treatment carefully. (author abst.)
