| Abstract;Cytomegalovirus (CMV) infection is commonly seen in immunocompromised patients. This time, we experienced gastrointestinal perforation due to CMV infection in a patient with malignant lymphoma during chemotherapy. A 75-year-old man complaining of abdominal distension, was diagnosed as having primary gastric malignant lymphoma. He was admitted to the hospital to receive chemotherapy. The treatment was effective, hut right lower abdominal pain appeared. Abdominal CT scan showed perforative peritonitis. GIF showed no apparent perforation. An emergency laparotomy was performed with a diagnosis of small intestinal perforation. On entering the peritoneal cavity, a perforation 5mm in diameter was recognized at the ileum end, and the jejunum at the site of 30cm proximally from the Treiz' ligament was almost perforated. Ileocecal resection, partial jejunal resection and drainage were performed. Pathological findings revealed perforation duo to CMV infection. Leakage was seen on the post-operative day 9, and he died from sepsis on post-operative day 22. Gastrointestinal perforation duo to CMV infection is very rare, and it is the tenth case in Japan. It is important to make early diagnosis and therapy. (author abst.) |
