Abstract;A 29-year-old woman who had noticed an abdominal tumor was seen at our hospital. A tumor the size of a child's head was palpable in her left upper abdomen, and the tumor moved to her lower abdomen during her hospitalization. A CT examination showed a giant multilocular cystic tumor with a solid component occupying the area from her left upper abdomen to her pelvis. An omental or mesenteric tumor was suspected because of its mobility, but characteristics similar to those of a pancreatic mucinous tumor were also present. However, the top of the tumor was located in front of the stomach, so a pancreatic origin was ruled out. The tumor was finally concluded to have originated from the liver because an angiogram showed that a branch of the left hepatic artery was feeding the tumor. Under a diagnosis of hepatobiliary cystadenoma or carcinoma, an operation was performed. The tumor arose extrahepatically from the lateral segment of the liver and was extripated by a hepatectomy. The tumor was 22 cm in diameter and weighed 1955 g. The histopathological diagnosis was hepatobiliary cystadenoma. A preoperative diagnosis was difficult in this case because hepatobiliary cystadenoma is a rare neoplasm and the present case exhibited marked extrahepatic growth. (author abst.)
