Abstract;A 59-year-old man was admitted to the hospital because of epigastralgia. Ultrasonography showed a mass lesion in the pancreas head region. Endoscopic retrograde cholangio-pancreatography showed a dome-like protruding tumor, 2.5 cm in diameter, in the lower bile duct causing obstructive jaundice. EUS showed diffuse hyperechoic spots inside the tumor, considered to be different from common echo-pattern of adenocarcinoma. Although angiography or abdominal CT failed to reveal abnormal findings, angio-CT revealed the tumor on delayed phase, which seemed to invade the pancreas. He underwent a pyloruspreserving pancreaticoduodenectomy. The specimen revealed a protruding white tumor, 2.6 * 1.5 * 1.5 cm in size. Histologically, the tumor was composed of sheets of small tumor cells. Chromogranin and Grimelius stains failed to demonstrate positive material in the cells. No neurosecretory granules were detected in the cells on an electron microscopy. Since these examinations did not demonstrate the differentiation of neuroendocrine cells, the tumor was diagnosed as small cell type undifferentiated carcinoma. The patient died of liver metastasis and wide spreading lymph node metastasis 6 months after the operation. This unusual case of undifferentiated carcinoma arisen in the bile duct is presented.
