Abstract;Hormone replacement therapy (HRT) is a therapy to supplement estrogen to perimenopausal and postmenopausal women to relieve their climacteric symptoms and has been used with an implement to reduce coronary heart disease. In fact, many American and European epidemiological studies reported that HRT halves death rate due to coronary heart disease (CHD) and promote the quality of life in senile women. However, recent prospective clinical studies revealed that HRT is not effective to prevent primary and secondary coronary events, and rather increases venous thrombosis and stroke. Thus, very recent announcement from American Heart Association says that HRT should not be used for the prevention of CHD. The negative effects of HRT concerning development of CHD may be caused by the increase in triglyceride (TG) with a regimen conventionally used, conjugated equine estrogen (CEE) and medroxyprogesterone acetate (MPA). The increase in triglyceride results in the production of very atherogenic lipoprotein particles, small dense LDL. Japanese women are less apt to suffer venous thrombosis and CHD compared with Caucasians, however, HRT should be prescribed with special cautions. For example, half dose of CEE plus MPA or transdermal estradiol may be safer than the conventional dose of CEE, since the former regimens do not increase TG so much. (author abst.)
