Abstract; We report a case of knee extension contracture without trauma or injection episodes. A 37-year-old woman admitted with a 10-year-history of progressive pain in the right anterior thigh muscles and difficulty in knee flexion. MRI showed thickness of the right tensor fascia lata and the anterior deep fascia. Biopsy revealed apparent thickness of the fascia and fibrosis of the sartorius muscle. The patient underwent partial resection of the anterior fascia, sartorius muscle, tensor fasciae latate and rectus femoris muscle, resulting in desappearance of knee contracture and thigh pain.
