| Abstract;A patient with aseptic effusion of the elbow joint induced by a poly-L-lactic acid(PLLA) Pin is reported. A fifteen year-old boy, a baseball pitcher,was diagnosed with osteo-chondritis dissecance of his dominant elbow and received internal fixation with a cancellous bone graft using PLLA pins. Four months after the operation, pain and swelling of the elbow appeared. Serous joint fluid was obtained by aspiration of the elbow joint. MRI showed joint effusion of the elbow. There was no evidence of septic arthritis. Arthrotomy of the elbow joint revealed protrusion of a PLLA pin, nonunion of the osteochondral fragment, and marked synovitis. The PLLA pin and osteochondral fragment were removed and synovectomy was performed. Soon after the second operation, the joint effusion disappeared. The pathological diagnosis was aspecific synovitis. Pathomechanisms of aseptic effusion of the elbow joint induced by the PLLA pin are also discussed. (author abst.) |
