Abstract;Tracheo-esophageal shunt phonation and esophageal phonation have been the most useful methods in alaryngeal vocal rehabilitation. Shunt phonations is common in both Europe and the U.S.A. The present study indicates that the time required for the acquisition of esophageal phonation is reduced when patients have already started shunt phonation. Therefore, shunt phonation is the first choice for patients just after total laryngectomy to acquire phonation quickly. They can then additionally obtain esophageal phonation in a shortened period. This study also demonstrates the importance of conservation of the posterior pharyngeal wall and the hypopharyngeal muscle for neoglottic formation and a better quality of shunt phonation after surgery, including laryngectomy, for patients with hypopharyngeal cancer. In addition, this study also discusses convenient secondary shunt procedures and front-loading replacement procedures for in-dwelling voice prostheses (Groningen). It is important to enlighten the public as to the existence of alaryngeal patients and alaryngeal phonation, as well as to pursue the acquisition of quick, easy, and high-quality phonation for a better QOL for alaryngeal patients.
