| Abstract;A 42-year-old male developed lassitude, loss of sense of distance, unstable gait and difficulty in speech, followed by right hemiplegia 3 weeks later. He had had diabetes mellitus since 10 years before. Ophthalmological examinations showed bilateral blepharoptosis, complete ophthalmoplegia and right homonymous hemianopia. Magnetic resonance imaging(MRI) showed lesions in left occipital and temporal lobes. He had elevated lactic acid in the blood, ragged-red fibers by skeletal muscle bipsy, and point-mutation of mitochondrial DNA. These findings led to the diagnosis of mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes(MELAS). (author abst.) |
