Abstract;The patient of this case was a 46-year-old Thai man who had cold-like symptoms. Three days after, he visited a local doctor with complaints of cough, abdominal pain and diarrhea. An infiltration shadow was found in the left middle subpulmonary field and antibiotics were prescribed. Respiratory distress developed in the midnight on the same day, resulting in emergency admission into ICU. Thus, treatment with Imipenem/Cilastatin sodium and Minomycin was started as an empirical therapy. Severe hypoxia and ventilation impairment was noted even after tracheal intubation. He developed anuria in addition to renal functional failure since admission and a lowered level of consciousness. Moreover, respiratory and circulatory dynamics became worse, resulting in death on day-2 of admission because of multiple organ failure. Thereafter, penicillin-resistant pneumococcus was detected in the nasal cavity, pharynx and spur, but anti-HIV antibody was negative.
