Abstract; The patient of this case was a 56-year-old woman who had headache from the morning on the day of admission. Her headache was not relieved by taking analgesic drug, resulting in a diagnosis of subarachnoid hemorrhage at department of cerebral surgery in our hospital. On the day of admission, she underwent emergency operation for left cervical artery-posterior communicating artery clipping. Anesthetic control was made with general anesthetics, but perioperative systolic blood pressure was increased to 180-200 mmHg. So, intermittent IV injection of nicardipine was needed. Although blood pressure was controlled with its continuous injection after the operation, she was attacked by hypertension and pectoralgia. Then, brown cytoma was revealed by close examination and she underwent right adrenalectomy, leading to discharge after favorable clinical courses.
