Abstract;Emergency surgery was made with a 68-year-old man who presented with epipharynx-derived malignant multiple brown cytoma and its bone metastases to lower thoracic spine and lumbar spine. Epidural anesthesia and myelosubarachnoid one were thought not suitable because of bone metastases. In addition general anesthesia through tracheal intubation was also unsuitable due to a massive tumor in the epipharynx. Therefore, monitored anesthesia care with combination of propofol and fentanyl was chosen. The surgery was terminated without trouble. Neither of perioperative arousal nor complication appeared. It was indicated that monitored anesthesia care is available for patients whose sympathetic nerve activities are enhanced by surgical treatments.
