Abstract; The patient of this case of emergency ophthalectomy was a very old woman at age 100 with dementia who had past history of bronchial asthma, hypertension and cerebral infarction. Without pretreatment, anesthesia was induced with oxygen, sevoflurane and maintained with atropine, sevoflurane and fentanyl after administration of 4% lidocaine and ephedrine. Her circulatory conditions were kept stable and the postoperative courses were good, resulting in discharge. For emergence anesthetization of very old subjects of which preoperative assessment is difficult, it seemed very effective to induce and keep anesthesia with oxygen and sevoflurane in combination with i.v.injection of Fentanyl.
