Abstract;The subject in the title was outlined. For the beginning, a blood pressure distribution chart of examinees in the first medical examination conducted in Sendai social insurance hospital was introduced. The hypertensive patient was 33% of all patients, and 36% of it was under the hypotensive drug treatment. The frequency of intractable hypertension (RH) in Japan and Europe and America was explained, and it was guessed that 5 to 10% of the hypertension patients was RH in Europe and America, which was lower than Europe and America in Japan. The cause factors of RH were shown in a list, which showed that the cause was diversity. It explained the disease state and diuretic (I) of RH and described that the addition of I was a basis when the hypotensive drugs of over three types with different pharmacological actions were used, because there was retention of fluid in the foundation in RH. The use of I was described, and the significance on the combined use of spironolactone in RH treatment was also described.
