| Abstract;The case was a 75-year-old man, who was in the advanced stage of a renal cancer. The role of a pharmacist in the period from the hospitalization to test outing was reported centering on the joint conference (J) of doctors, chief nurse and pharmacists. The doctor explained to the family that the recovery by the treatment could not be expected but the family desired the home remedy. Accordingly, a plan from above described J to the leaving hospital through outing and being away overnight was drafted. The examination content was illustrated and it was decided that the outing would be examined after the normalization of antipyresis and the screening test value. There was a report that each examnation result was normalized in final J from the doctor side, and the patient's outing became possible after the confirmation of side effects by the pharmacist, etc. In the future, the home treatment seemed to occupy the important position, and it was considered that making of a system for cooperation of the medical workers and continuation of information exchange was necessary. |
