| Abstract;Misdiagnosing case in ambulatory treatment at night was introduced. The case was an obese woman of 54-year-old, whose chief complaints were chest pain and dyspnea in walking. The duty doctor diagnosed it as conventional frequent intercostal neuralgia only by an X-ray photograph, and carried out continuous infusion and dosing and made the case go home. The case caused the dyspnea on the way home and died after 2 hours. The cause of death was determined to be pulmonary thromboembolism from the autopsy. It was proven that there was an opinion of above described disease before the death as a result of comparing with a chest radiograph of 5 months before in the hospital, and that the duty doctor overlooked it. However, it was judged that the lifesaving was impossible even if the case was diagnosed and treated rightly and there was no direct causal relation between misdiagnosis and the death. It was indicated that the correspondence which was not caught in the medical history was important, because even the chief complaints were the same, they were not always the identical disease. |
