Abstract;Bilirubin in the hepatic function disorder was outlined. Following 7 items were explained on metabolism and pharmacokinetics of bilirubin (B) : 1) Generation of B, 2) Blood B, 3) Intake of B to the hepatocyte, 4) Transportation in the hepatocyte, 5) Glucuronic acid conjugation, 6) Excretion to bile canaliculus, 7) Metabolism of B in bile duct and intestine. Membrane transport protein which existed in the hepatocyte which fulfiled the important role in the pharmacokinetics of B was illustrated. Following 4 items were explained on the reading of the blood B value, and the relevance to the disease state was explained : 1) All B, 2) Indirect B, 3) Direct B, 4) Four fractionation of B. It was explained that the stage and degree of the hepatic function disorder could be read out considerably accurately by understanding the pharmacokinetics of B at the molecular level.
