Abstract;The clinical application of nCPAP is recognized as one of the most important advances in the treatment of obstructive sleep apnea-hypopnea syndrome. nCPAP is a simple treatment which can be started easily and safely. The health insurance application criteria in Japan judges the indication for nCPAP by the apnea hypopnea index (AHI). Also, the confirmation of the existence of a sleep disturbance by polysomnography is required. If this is improved by the use of CPAP, the patient can continue the treatment for one or two months. We have experienced more than 1000 cases of diagnostic sleep study since 2002, and have followed up nCPAP in about 250 patients in our out-patient department. The biggest problem of nCPAP is the difficulty of the continuation of nCPAP treatment. The mechanical noise of CPAP was a problem that 51% of the patients complained of our patient survey. However, we must consider the possibility that this phenomenon may not be evaluated objectively. There is a pattern of every patient in terms of adherence, and this pattern can not be evaluated according to an average time. Doctors should use CPAP with a built-in adherence memory system for objective evaluation. Of course, the patient's own will is most important in the continuation of treatment for nCPAP. The emotional status of the patient who has had nCPAP prescribed is not simple. Many patients feel a depressive mood, and this disturbs CPAP continuation. Various problems are seen in nCPAP users or their families. However, there are not many problems connected with the interruption of the treatment. Actually, adherence to nCPAP therapy at Iwate Medical University after the national health insurance application was more than 90%.... (author abst.)
