Abstract; We report a rare case of intratendinus ganglion in the extensor Pollicis longus (EPL). A 41 year-old man had a buiging mass at the right dorsal hand over the past six months. The mass gradually became larger and larger, and he had a pain at thumb motion. At surgical exploration, we found the cystic lesion within the substance of the EPL tendon and excised it. Pathological analysis showed that the cyst was multilobulated and contained bloody mucoserous liquid, and the fibrous wall of the cyst was lacking of synovial lining cell layer. After the operation, pain was relieved and there was no recurrence at postoperative six months. (author abst.)
