| Abstract;Whether multidimensional assessment of the psychiatric and psychological aspects of an abused child and its abusive caregiver is clinically useful and significant for obtaining a comprehensive profile of child abuse was investigated. A six-year-old boy, who had been physically abused by his parents, and his mother were studied. Physical abuse had been confirmed by a child guidance clinic. A multidimentional assessment was made by means of the following established measures. For the boy: 1. The psychiatric diagnosis was based on his scores for the Diagnostic Interview Schedule for Children-IV, DuPaul's scale for parents and teachers, and the Child Depression Inventory. 2. For the psychosocial measurements the Wechsler Intelligence Scale for Children-R the Child Behavior Checklist (CBCL), the Teacher Reporting Forn, and the Self-perception Profile for Children were used to assess the child's cognitive level behavioral problems and his self-esteem. For the mother: The Semi-structured Clinical Interview for DSM-IV axis I disorders was used for the psychiatric diagnosis. The General Health Quentionnaire, 28 items, was used to assess her psychological-physical condition. Disruptive behavior disorder and depression were confirmed in the boy. Moreover, broad band behavioral problems were identified by the CBCL, not only were externalizing but internalizing ones, even though he manifested no obvious internalized psychological problems. Furthermore, low self-esteem was confirmed. The mother was found to have suffered from post-traumatic stress disorder accompanied by a chronic anxiety state and physical exhaustion for which there had been neither short- or long-term intervention. Interaction between the child's and mother's psychiatric disorders was considered as a factor that maintained child abuse.... (author abst.) |
