| Abstract; We report on the therapeutic process for a very low birth weight infant of twins, and his mother with a tendency to abuse him. PDD was diagnosed at age 3 years and 4 months. The mother had gone through many crises: infertility treatment, long-term hospitalization, the delivery of twins, and internal conflict with her mother about the infant's needs. These factors seem to have triggered the vicious cycle of her abuse of this child. Consultation with the mother was begun during the infant's NICU stay. Later, more aggressive psychotherapy was deemed necessary for both the mother and infant, and he agreement was obtained. In the initial interview, empathy was shown with the mother's negative feelings for her child and eventually she accepted him. Development of a positive relationship between them was helped by listening to the mother's stories of the internal conflicts produced by the child's behavior. This early intervention improved the mother-child relationship and promoted the child's development. (author abst.) |
