Abstract;A 24-year-old, single undergraduate woman with anorexia nervosa of the binge eating/purging type was admitted to our emergency unit because of a femoral neck fracture suffered while jogging. She was referred to a psychiatrist on the second day after admission. Initially, she was extremely reluctant to undergo any psychotherapeutic treatment due to low self-esteem and nihilistic thinking during the chronic course of her eating disorder and having undergone multiple hospitalizations since age 14. We therefore focused on her low self-esteem rather than her abnormal eating behavior during the cognitive therapy and motivational interviews. Over the next two years, her eating behavior gradually improved, and her bone mineral density increased (from 0.69 to 0.81g/cm2). She finished graduate school, and began start working the next spring. Focus on enhanced motivation and on the individual's low self-esteem as well as on skeletal complications, during psychotherapeutic treatment is important, particularly when treating early onset and chronic patients.
