| Abstract;Postoperative endophthalmitis developed in two cases out of a consecutive 13,024 cases of cataract surgery during 16 years through 2003. One was a 72-year-old female with chronic dacryocystitis. Two weeks after surgery, corneal ulcer formed at the site of control suture in the lower limbus that later developed to endophthalmitis. The other was a 72-year-old male. Endophthalmitis occurred after transscleral fixation of intraocular lens necessitating vitreous surgery. Results of intraoperative culture of conjunctival scraping showed that endophthalmitis can be prevented by better cleanliness of the surgical theater and by shortening the duration of surgery. The majority of microorganisms isolated from the conjunctival sac several days after surgery were the same as those present before surgery. Most cases of postoperative infection can be avoided by keeping the cornea free of wound, by selecting proper antibiotics before and after surgery, by keeping the surgical room clean, and by shortening the duration of surgery.
